More than 90 million Americans live with chronic diseases. The direct and indirectcosts of chronic diseases are in the trillions of dollars per year. This UCSF Osher Mini Medical School series, modeled after courses in the medical and pharmacy school curriculums, explores specific patient-practitioner relationships affecting patient outcomes of major disease. Special emphasis is placed on the pharmacist care service model and interprofessional care.
